1/ Follow the steps and fill in the declaration form to the end, then validate the request.
2/ A confirmation e-mail summarizing your declaration will be sent to you automatically,
3/ A second automatic e-mail indicating your file reference will be sent to you a few days later,
4/ Once your claim has been processed, you will receive an e-mail from the administrator in charge of your file. All requests relating to your claim must be made by e-mail. There is no need to contact us again, or to make a new claim.
5/ For medical expenses, we only cover the remaining out-of-pocket expenses. Please contact us by e-mail after your health organizations have intervened.
